If I understand it correctly, the initial letters scheme only indicates the source.

In fact, that was my breakthrough. Clues 15, 16, and 17 gave the letters R, A, K for example.

The tomfoolery gives he source from the 13 normal clues first letters, but doesn't impact the grid at all. Not easy to figure out from the preamble.

Phew, finally got there (thanks to some good hints here) after wasting hours on completely on the wrong track, looking at an ‘infernal’ work with a 14 letter creator. Thanks Gnomish, you really had me foxed.
