The clue you refer to is “Twisted paper that Charlie leaves under golf bag”. I’m not sure that they are strictly equivalent, but the middle three letters of “paper” can mean “a coarse, clumsy or stupid person”, and “Charlie” can mean “an inefficient, ineffectual person a fool”. So take the three middle letters out of “paper” reverse (“twist”) them, put G for golf on top, add a missing letter, and you get the obvious “bag”.
Not sure what the anatomical deformity you refer to is, but looking at the original work, I can see an asymmetric treatment at the foot of the image, which may prompt the asymmetry at the foot of the grid.
