The synonym might also describe someone who administers vaccinations...

I too cannot figure out 4d despite all the hints previously given—I understand the hints in terms of the cryptic process involved, but I think my lack of comprehension around the theme is working against me. I am scratching around the edges of understanding it but I don't have enough fully-parsed thematic material of what I presume to be the second group, that I can't seem to reverse engineer it. This is definitely the most stuck I've been in my relatively short Listener career to date...

Mullinger, in 4 down replace the JA of jar with an anagram of NEIN.

Thank you Jeg. I can rest easy. Didnt have the correct synonym for rabble. đź‘Ť
Chrisremo if you have the word which connects the answers to one of the sets think of an old song (music hall) and revisit the crossword. Particularly look at 10 down what does the entry have in common with the answer and more importantly what are the differences. Hope this helps without spoiling your fun

Cheers mullingar—the song I had in mind seems to be the correct one, and I had an inkling as to what to do with the second set, but what I didn’t pick up on until after my most recent post was the linguistic element. Time to dive back in, I think I’ll be able to close this out finally…

Enjoyed that a lot, thanks Elfman.

Loose ends for me :

Still don't understand 4d, despite all the previous posts. I have the answer, understand the substitution and the definition, but I cannot find any synonym of the first word that contains the 'j' word. Maybe someone could tell me what the fall synonym is?

Does one of the first substitutions involve replacing a single letter, rather than words? Seems inconsistent with the others and the song title, but my grid works with a single letter replacement.

Thanks

Chambers gives rabble as "to gabble" and if you look up gabble in Chambers thesaurus a 6 letter word beginning with J is there (as I noted earlier you can also use this word to describe someone administering a vaccination)

If you are referring to 11a, I interpreted it as using the Scottish words but I suppose a simple "one letter for another" could also work as an interpretation.
