61d - this is a triple definition - 1) the "poker" is one used in fireplaces, 2) a "difficult problem" and 3) a file containing memoranda, short notes etc

Finally staggered across the line thanks to some help from pgbj and Brendan, but can't parse 50. It can't be anything else, and the definition seems obvious, but can't make any sense of the wordplay.

castafiore - it means the first three words whether you add the "bit of tooth" or not ...
