I seem to have found this more difficult than some, but got there in the end. Fairly enjoyable.

For 18d I think “extremely self-satisfied” would be more accurately written as “the most self-satisfied”

Unlike most, I found this a bit of a struggle, especially in the SW ... Not helped by Everyman's dodgy grammar - Who ARE, not Who's! On the other hand, the homophones raised a couple of smiles.

Cosmo ... Think of rough Mediterranean wine (not Sancerre), then move the middle letter to the end.
