Aaah, yes I'd overlooked 18a, just realised I'd not parsed that either :-/ I can see the wordplay so now just need the missing piece, many thanks for putting me on right track muraria. Still don't see how definition = entry for 42d, though!

No worries cb, 18a was my LOI, it's one of the more obscure Asian "stan" thematic entries. Entry to 42d is part of table tennis.

Phew! That was a difficult solve. 18a was also my LOI although it did make me smile when the penny finally dropped - "cost to reclaim" brilliant 😂

Well, had a shower, made a coffee and then found the parts to be highlighted pretty much straight away, and, for once, I actually get the point of the title! Trust me - no-one's more amazed than me 😃
