...sorry, thought you meant the letter after the dropped 'r'. The 'r' leaves an anag of a word that derives from 'matelot'

I'm sure you must have the 4 letter one in 37d

Got there thanks! Interestingly Chambers references it from matelot rather than the other way round. Will grid stare tomorrow for the end game.

Finally sorted everything out.

I was stuck for a while, after I moved the 3 lower letters up, trying to find how they related to "when the trouble might have started" - then I noticed what was written underneath - brilliant!

What an excellent puzzle.

Thanks, Brendan. We're bound to get less impressive puzzles at some point, but 'less impressive' is a matter of taste, personal opinion. Last year we had several Inquisitors that I heartily disliked, yet the majority of solvers really enjoyed them, enthused about them.
