I'm a bit stuck, i have everything except the uncrossed letter in 38d. The gentlest of hints would be appreciated!

The solution to 38d is a two-syllable word. Regard the syllables as separate words, one of which gives an indication of how the other should be treated for grid entry.

Andyp - thanks for the offer - but after watching The Gold, all fell into place - the transformations aren't as sophisticated as I'd assumed - and, crucially, the dictator ain't Tito!

Post 44 states 16d should be split 6,1,2,1 but that leads to an instruction with 2 abbreviations which is subtly different from the other thematic entries. Surely it should be read 5,3,1,2? That leads to a perfectly acceptable solution using just the 2 abbreviations. I stated earlier that there is no ambiguity - perhaps that was a bit rash?
