Spinynorman1, it's an anagram whose grist is (2 words from the clue - 4 and 3 letters respectively) + (the first letters of 2 words from the clue).

Thank you chrise and geeker! Could have stared at that all day and would never have gotten it.

Started after lunch as usual and had got three before resorting to the forum. FOI was 28ac. Geeker is right, it is very hard and I'm not enjoying it. I'll give it a couple more hours and then may give up.

I can't think of any football managers, but I'll have another go at the difficult anagram in 9,1 now I know the definition. Many thanks to ChrisE for his helpful comments.

Back again, but not much further. This is the third time I've written this as I've been having trouble with the captcha.

Please can I have some hints for 17ac, 12ac and 20d. I have no crossers in any of these.
I'm four letters short for the anagram in 9,1. What are they? I counted wrong at first, I thought I just had to add "don". I'm assuming the definition for 19,18 is "Previous Issues", but I'm none the wiser.

19a is anag of LIRT+MS+DENISE+VAN+OUTEN
(first part is FLIRT with first letter dropped).

I had a lot of trouble with this. Based on my experience, I suggest getting the second word (see hints above re. uni), then the first word will be easier to figure out.

17 is one letter for American then four letter word for fight (e.g boxing) meaning approx.
12 is four letters for tablet and two for ‘that hurts’. Found on a bed
20 usual single letter for hospital with a six letter word for heart (informal) around it, meaning bigger
