I think12 and 25 are just weak cryptic definitions. An ice skater could make figures of eight on a frozen river; FAIL SAFE means it doesn't kill anyone if it stops working.

4 THE U BLAME anagram gives EAT HUMBLE, but I don't see why confusion gives PIE

8 extensive = HIGH ?
