Back again needing help please. I have completed the grid and the two phrases but have no idea what I am looking for in the grid.....come on guys...please

I've 'noted' 12 -letter combinations that may give a "theme" but I've no idea how they fit together!

That should be 12 2-letter combinations....

Each character has a dedicated row - move up and down diagonally and L to R.

I have almost filled the grid, but cannot parse two clues.
46a, "issues uncovered after equitable ownership" i have use_ ( I can't see how to add a d to the clue here)
17a Image of media item in freak convulsion (+ n), I have e_it, obvious entry is emit but can't see why. I'm thinking medina, possibly, giving an Arabic meaning?

'dafter' gives the anagram indicator.
'median' gives the middle letter of 'freak'.

...but the answer isn't 'emit' - you need a 3 letter synonym for 'convulsion.'
