All done, and really very straightforward, except

28ac makes no sense to me

I can’t see 40ac

And I’m missing one of the 12 alterations

28 requires you to focus on PRIME

For 40 - look at the numbers 26 and 24 rather than clues 26 and 24

Your missing alteration may appear once you sort 28ac - hard to know without knowing the 11 you have found

I have been looking for one more alteration too, and I think the light bulb finally went on. Is 1a altered?

Mine is and it was my twelfth. Doesn't make it right though

First day off the drink for a long while yesterday. Result - sailing through the first listener of the year! Or it was pretty straightforward. Early PDM with 1ac being last one in. Tried to justify a synonym for song in it for a long time before I realised it was the 12th replacement. Surprised by 39ac, but anyway. Happy new year all!
