The only remotely interesting thing about this puzzle was my (American) wife’s horrified reaction to the thought that anyone would regard 19a as “of a kind” with the othe unclueds. It would take nothing short of a Constitutional amendment to make that true.

wow! didn't even notice when I solved. Is Doc his own editor? (isn't there an adage about doctors shouldn't be their own doctors?).

and I don't understand wp for the "Call me this..." clue.

Jack, I thought exactly the same about 19 A. I felt this was a pretty awful puzzle from start to finish - and a very short 'start to finish' at that.

For what it's worth I read the wordplay for 40 across as an anagram of 'Call me this in' minus 'Clint'. I agree with the comments re 19 across and the opinions expressed about the puzzle itself.

thanks ap! seems obvious in retrospect. As for 19a -- at least two well-known purveyors have items with that name. So acceptable in my view.
