With Dumpynose (Pseudonym !) you usually get lots of anagrams & find that the crossword title is a big hint-
i.e PALSY = Plays

I don't get the wordplay for 20 A but found 8 Down straightforward - just two words which sound the same, only one of them means tot(add!)

20ac is an alternative spelling of lief, meaning sweetheart, plus the IVR for Lesotho.

Sorry, this might seem obtuse - but what does 'plus the LVR for Lesotho' mean? I've got the actual answer, via Chambers, but am just slightly puzzled.

The title, PALSY, is more subtle than a simple anagram. Palsy is a shaking ailment. If you 'palsify' PLAYS you get PALSY!
