Some pretty weird solutions in this puzzle. I admit I can't parse 17 despite your hints, and as for 19a, I'm fairly sure I have the right crossers but can't find any word that fits - maybe I'm being dense?

Brendan - I also thought 2d required a homophobe indicator in the brackets, but a quick Google of the NATO alphabet suggests it doesn't, so it seems I've been spelling it wrong all these years.
COD 15a.

Neville - 17a is an anagram with one letter removed, as instructed, with the answer simply describing the state of playing cards before a game begins.

Apologies, Neville. I've just realised you meant '17' in 2d! Count through the alphabet.

Thank you Phil1000 and Yevrah - I had a wrong crosser in 19 but I should have got the reference as I'm in French (and in France - where it's snowing!)

Nunoesque, interesting interpretation, didn’t occur to me, but it seems perfectly valid.

Rainman and Jono - the car was how I arrived at the answer to 2d. Parsing the bracketed bit came (much!) later.
