'Geisha' fits but I can't see why.

Begins with 's'. Sometimes I just sense I won't be able to get an answer and this is one of those times.

Thank you Candledave. I think using 'city' as a definition is so vague as to be almost pointless, like using 'name' or 'book'.

I was late to this but thoroughly enjoyed it. There was, I agree, a fair smattering of place names and locations as definitions, but all were so generously checked by crossers that I had no quibbles at all. I was unaware that Rigoletto was a hunchback, but had no problem solving the clue.

As far as I can see the answer to 3D

With which condition one doesn't normally feel great? (11)

has to be "MeGaLoMaNiA" (crossers in UPPER CASE) but I cannot justify and it appears to be the opposite meaning to the clue.
What do you think?

I agree with earlier comments that this crossword is unfair if one can justify only by the crossing clues.
