Bar = cafe + o = 15th letter + que = what in French

v = versus (against).
I assume “one wallowing” is hippo, but I don’t see the rest.

Thanks geting. Don't like eye = bias (really?) or fifteenth = o without any indication that the ordinal refers to the alphabet.

Part of Hippocratic oath is patient autonomy , so I guess it's a cryptic based around that and the fact a hippo wallows , maybe?

Not sure of this but I think...
