I think it must be riflemen. Rife - men, left inside. Are you sure of the a? (rife meaning widespread)

From the clue, it looks to me like the answer should be riflemen.

Thanks guys - I had Macaroni for 42d but I can see now it should have been Rigatoni with Riga being the capital.
