21 Across - Fish leaving river - cost measured in lives? (4)

I know the answer is TOLL, and that is of course "cost measured in lives" but I can't see how we get there from "fish leaving river". Can anyone tell me what I am missing, please?

Baleful has Do replacing BA
Bewitch is Brew less R plus Itch (long). Verb to make intoxicated
Ova is a homophone for Six balls (deliveries) in cricket

AB (short for Able Bodied) seaman
OS short for Ordinary Seaman

Looks like Andalusia, but I can't see the "mostly luxurious".

Lus(h),luxurious (mostly/the word gets cut) And, with, A in clue Lus+ I, island +A,area.
