I thought either Mr Bean or Mr Benn, but didn't make the "head" connection

A refresher is a payment made to counsels in legal cases to ensure they continue to work for the client - so cryptically a refresher course could be something that trains a counsel/barrister how to elicit refresher payments, I guess.
