Finished (unusually easy for a Philistine, I thought), but one unparsed. Could someone point me in the right direction, please?

4,19 Unintended consequences of local alert (10,6)

First two words give the definition, but I can't see any relevance of the last three. (I haven't given the placed letters on purpose, as they might give too much help on other clues!)

4 19 Is the first word not an anagram of local alert?
