Finished this, but not entirely clear on the parsing of
2 Down:- "Back upset, swallowing medicine? On the
contrary, here's a difficulty", I have PITFALL for the
answer but cannot see where the medicine comes
into it.

It's AFT "upset" in PILL (medicine), though the containment indicator "swallowing" must be distorted for it to work the right way round.

Actually, "on the contrary" does get the conatinment the right way round, I see now.
