Oh thanks very much Mamya - I stupidly had never thought about doctors being an anagram indicator - was thinking neuro but couldn't get why there was no letter e!
Finished now, my bed is calling me...........

I'm adding a clue that I'm not sure about,, having
finished the puzzle. 46 Ac is:- "What mad chap
sees then does for Exmoor fauna, say". (3,4)
I have RED DEER as the answer, but cannot see
where the mad chap comes into it.

Mad chap sees - RED
Does (female) DEER

If you're a "mad chap", you see RED.
Doe a deer a female deer.
