Clue please?

"As doctor probes a large hole in head, start to scream"(10)

palpitates

al (a large) pit (hole) in pate (head) + s (start to scream)

Palpitates

I think the compiler's made a mistake!
A doctor feeling someone is palpating them. Something that palpitates has a throb eg a heart.

So pates outside a large pit leads to palpitates which is not "as doctor probes"
but palpates (as doctor probes) is outside "it" which is not a large hole in the head.

What do you guys think?
