Move the H in shin!

I'm a bit puzzled about 32ac as well. Moving the h in shin doesn't give the answer. Moving the s does, which suggests that "head" refers to the fist letter of shin, but I can't really see how that squares with the clue.

Lower part of leg, with its head (ie the head of "shin", not of "it") ticked inside. Thank you Nikki.
