It is in Chambers dictionary but there is no indication as to where the phrase has its origins.

Perhaps someone knows where it comes from.

Thanks everyone. I'd thought of Cousin German from the wordplay but have never heard it used or come across it in a crossword before.

If you google it there's a hyphen. I'd never heard of it either.

The word "german" (without the capital) is an adjective meaning "of the first degree". You therefore have "brother-german", "sister-german", "cousin-german", and so on.
